Just some few remarks, if you don't mind.
1- Testing should cover all aspects already taught in class or having experience with (diagnostic assessment) ; i.e, comprehension, grammar, sound system and the like.

2- Questions / Instructions should be set well so as the learner finds his way through, i.e; keep to easy and clear instructions

For instance,

a- TASK 4- Instead of saying "find the plural of these nouns, you can say "supply the plural form of the se nouns" or just draw a table of two columns : "nouns" and "plural" and the learner will see the task differently.

b- TASK 5- When asking the testee to "find" the opposite of a word out of the blue, a cloud will overwhelm his/her mind at the time of the test. Instead, the following instruction can help much more by asking "Form" the plural of the following nouns or just supply an example then ask the learner to follow it. S/He will do it easily through inference.

c-TASK 9- The instruction used to target the sound system was about unclear. She could formulate the instruction as follows: "Classify the following words acoording to their "ch"/"sh" pronunciation.

d- TASK 10- The fact of asking the learner to complete sentences , you are testing his ability of dealing with the target language. I mean it is useless to be redundant when testing. Just have a look at TASK 3 and TASK 10. Doin't they look similar in their aim?
For that, I advise new teachers to set aims not only for the lessons and tasks they teach in class but even for homework , tests and exams. At that time, you'll feel how minute is the result you will get by (not "at") the end of the year.

I hope I haven't been so talkative as after having a glimpse on the paper, professionally, I couldn't keep these remarks for myself.
